About the Role

We are seeking a skilled Community Health Nurse to join our team at North Cumbria Integrated Care NHS Foundation Trust. As a key member of our community nursing service, you will play a vital role in delivering high-quality patient care in a variety of settings.

Job Summary

This is an exciting opportunity for a motivated and compassionate nurse to work in a dynamic and supportive environment. You will be responsible for assessing, planning, implementing, and evaluating patient care to meet their individual needs. Key Responsibilities

  • Assess patient needs and develop personalized care plans
  • Deliver high-quality patient care in accordance with professional standards and guidelines
  • Work collaboratively with multidisciplinary teams to ensure seamless patient care
  • Communicate effectively with patients, families, and carers to promote understanding and involvement in care decisions
  • Monitor and evaluate patient progress, making adjustments to care plans as needed

Requirements

  • Degree-level qualification in nursing (Level 1)
  • Registered with the NMC as a practicing nurse
  • Proven experience in community nursing, ideally in a rural setting
  • Excellent communication, interpersonal, and teamwork skills
  • Able to travel independently across a range of settings
