About the Role
We are seeking a skilled Community Health Nurse to join our team at North Cumbria Integrated Care NHS Foundation Trust. As a key member of our community nursing service, you will play a vital role in delivering high-quality patient care in a variety of settings.
Job Summary
This is an exciting opportunity for a motivated and compassionate nurse to work in a dynamic and supportive environment. You will be responsible for assessing, planning, implementing, and evaluating patient care to meet their individual needs. Your excellent communication and interpersonal skills will enable you to work effectively with patients, families, carers, and other healthcare professionals to achieve positive outcomes.
Key Responsibilities
- Assess patient needs and develop personalized care plans
- Deliver high-quality patient care in accordance with professional standards and guidelines
- Work collaboratively with multidisciplinary teams to ensure seamless patient care
- Communicate effectively with patients, families, and carers to promote understanding and involvement in care decisions
- Monitor and evaluate patient progress, making adjustments to care plans as needed
Requirements
- Degree-level qualification in nursing (Level 1)
- Registered with the NMC as a practicing nurse
- Proven experience in community nursing, ideally in a rural setting
- Excellent communication, interpersonal, and teamwork skills
- Able to travel independently across a range of settings
