Job Summary

We are seeking a highly skilled and motivated Mental Health Specialist to join our team at Avon and Wiltshire Mental Health Partnership NHS Trust. As a Recovery Practitioner, you will play a vital role in delivering high-quality care to individuals with complex mental health needs.

Main Duties

The successful candidate will be responsible for:

  • Providing comprehensive mental health assessments and developing personalized recovery plans for service users.
  • Coordinating care and delivering therapeutic interventions to individuals with complex needs.
  • Working collaboratively with multidisciplinary teams to ensure seamless care coordination and effective risk management.
  • Developing and maintaining good partnership working with other services, including primary health care teams, inpatient services, and voluntary sector organizations.
  • Facilitating the development of complex crisis plans and advance statements, involving other agencies as necessary.

Person Specification

The ideal candidate will possess:

  • Current registration with the NMC or HCPC or SWE and commitment to CPD.
  • Diploma level/Degree in a relevant health profession.
  • Ability to deliver a range of key therapeutic interventions, including CBT, psychosocial interventions, and family interventions.
  • UK driving license with business insurance.
