As a Rehabilitation Assistant Practitioner, your primary responsibilities will involve working with individuals in their own homes, conducting non-complex assessments post-discharge from hospital, and delivering targeted rehabilitation programs to maximize patient independence.

Key elements of this position include working alongside existing practitioners and care support workers to facilitate discharge and prevention of admission. You will also be responsible for managing a caseload, providing assessment, rehabilitation, onward referral, and discharge for patients referred to the service. Regular supervision is provided, including peer supervision opportunities, and ongoing professional development is encouraged.

Required qualifications and skills include the ability to travel efficiently between work locations and patient residences, necessitating a full current driving license and the use of a vehicle.
