Job Summary:

This is a unique opportunity to work as a Complex Care Coordinator, overseeing care packages, supervising new staff, and maintaining relationships with clients. You will work closely with our Care Manager to ensure seamless care delivery and make a real difference in the lives of our clients.

Key Responsibilities:

  • Develop and implement care plans that meet individual client requirements, working closely with our Care Manager.
  • Supervise new care assistants through their induction, assessing competency and providing ongoing support.
  • Work with the care coordinator to maintain shift cover, ensuring continuity of care delivery.
  • Perform clinical duties, including personal care, medication administration, and meal preparation.

Requirements:

Minimum Level 3 Health and Social Care qualification, experience supporting those with disabilities and complex health needs, and a full UK driving licence and car. Strong communication, organisation, and problem-solving skills are essential.

What We Offer:

Competitive salary range £26,000 - £28,000 per annum, depending on experience. Ongoing CPD training, paid mileage, use of company mobile and app, and more.
