About the Role

We are seeking an experienced and skilled Senior Clinical Pharmacist to join our team in the Primary Care Network. As a key member of our team, you will be responsible for providing clinical leadership on medicines optimisation and quality improvement, and managing some aspects of the quality and outcomes framework and enhanced services.

The successful candidate will have a strong background in clinical pharmacy practice, with a minimum of 5 years post-qualification experience. You will have a deep understanding of the principles of evidence-based healthcare and be able to communicate complex and sensitive information to a variety of audiences.

You will work closely with our GP Practice Leads, Clinical Supervisor, and Greenwich Health Federation to deliver high-quality services to our patients. You will be responsible for managing medicines at discharge from hospital, providing patient-facing clinics for those with questions, queries, and concerns about their medicines, and working in partnership with hospital colleagues to proactively manage patients at high risk of medicine-related problems.

Key Responsibilities
  • Provide clinical leadership on medicines optimisation and quality improvement

Requirements
  • Minimum 5 years post-qualification experience
  • Deep understanding of the principles of evidence-based healthcare
  • Ability to communicate complex and sensitive information to a variety of audiences
  • Registration with the General Pharmaceutical Council
  • Membership of the Royal Pharmaceutical Society
