About the Role

We are seeking a skilled Palliative Care Nurse to join our community specialist palliative care team. The successful candidate will work under supervision to provide patient-centred, evidence-based specialist services for patients and their carers with palliative and end-of-life care needs.

Main Responsibilities

  • Take overall responsibility for the care of a caseload of patients with a palliative diagnosis, requiring specialist nursing care.
  • Offer the patient and those important to them the skills of a CNS in symptom control, social, emotional, and spiritual care.
  • Participate in maintaining and developing a quality service.
  • Ensure the delivery of agreed professional standards of care within their specialist area.

Requirements

  • Demonstrable experience of working with end-of-life patients and contributing to the quality and development of the service.
  • Demonstrable Degree level study or relevant post-registration experience/study.
  • Experience of working in the community.
