Oxfordshire Supported Care Coordinator Role

Job Summary:

As a Supported Care Coordinator, you will be responsible for overseeing a cluster of supported living services across Oxfordshire, focusing on delivering high-quality, person-centred care that meets the unique needs of each individual. This involves reviewing and implementing care plans, conducting risk assessments, and ensuring the ongoing training and motivation of your team.

Responsibilities:

  • Review and update care plans for service users to ensure they remain relevant and effective.
  • Develop and implement risk assessments to safeguard the health and wellbeing of service users.
  • Maintain open communication channels with staff to foster a culture of continuous improvement.
  • Act promptly in addressing any complaints or concerns raised by service users or their families.

Essential Qualifications and Experience:

  • Postgraduate degree in a relevant field, such as social work or healthcare management.
  • Prior experience working in supported living environments or similar settings.
  • Excellent interpersonal and leadership skills.
  • Able to navigate and adapt to changing circumstances within a dynamic environment.
