Parkinson's Disease Care Coordinator

As a Neurology Nurse Specialist at Herefordshire & Worcestershire Health and Care NHS Trust, you will have the chance to make a tangible difference in the lives of patients with Parkinson's disease and associated movement disorders. Our Integrated Neurology team provides expert care in community settings, and we seek an experienced nurse to lead our efforts in this field. About the Role:
This role involves conducting comprehensive assessments, developing personalized care plans, and collaborating with Consultant Neurologists to deliver high-quality patient care. You will also provide education, information, advice, and support to patients and their families in various settings, including community homes, clinics, and remotely. We require someone with excellent communication skills, who can work independently and as part of a multidisciplinary team. The ideal candidate will have a strong background in neurological rehabilitation management or neurology, with evidence of ongoing professional development.
Responsibilities:
• Conduct comprehensive assessments for patients with Parkinson's disease and related conditions
• Develop and implement tailored care plans in collaboration with Consultant Neurologists and other healthcare professionals
• Provide patient-centered education, information, advice, and support in various settings
• Work closely with the multidisciplinary team to ensure seamless care coordination and delivery
• Contribute to quality improvement initiatives to enhance patient outcomes and experience
Requirements:
• Registered General Nurse or Registered Nurse Learning Disabilities qualification
• Postgraduate training and significant clinical experience in neurological rehabilitation management or neurology
• Strong communication and interpersonal skills, with ability to work independently and collaboratively
• Proficient in prioritizing tasks, managing time effectively, and meeting deadlines
Salary: £40,000 - £50,000 per annum, depending on experience.
