Job Description:

The Integrated Neurology Team at Herefordshire & Worcestershire Health and Care NHS Trust is seeking a qualified and experienced Neurology Nurse Specialist to join our team. As a specialist nurse, you will work closely with patients, families, and healthcare professionals to deliver high-quality care and support to individuals with Parkinson's disease and associated movement disorders. About the Role:

This full-time position involves conducting comprehensive assessments, developing personalized care plans, and collaborating with Consultant Neurologists to deliver high-quality patient care. You will also provide education, information, advice, and support to patients and their families in various settings, including community homes, clinics, and remotely. We require someone with excellent communication skills, who can work independently and as part of a multidisciplinary team. The ideal candidate will have a strong background in neurological rehabilitation management or neurology, with evidence of ongoing professional development.

Responsibilities:

• Conduct comprehensive assessments for patients with Parkinson's disease and related conditions
• Develop and implement tailored care plans in collaboration with Consultant Neurologists and other healthcare professionals
• Provide patient-centered education, information, advice, and support in various settings
• Work closely with the multidisciplinary team to ensure seamless care coordination and delivery
• Contribute to quality improvement initiatives to enhance patient outcomes and experience

Requirements:

• Registered General Nurse or Registered Nurse Learning Disabilities qualification
• Postgraduate training and significant clinical experience in neurological rehabilitation management or neurology
• Strong communication and interpersonal skills, with ability to work independently and collaboratively
• Proficient in prioritizing tasks, managing time effectively, and meeting deadlines

Salary: £38,000 - £48,000 per annum, depending on experience.
