Job Overview
We are seeking a skilled and compassionate Mental Health Specialist to join our Community Mental Health Team. As a key member of our multidisciplinary care team, you will be responsible for assessing, planning, coordinating, delivering, and evaluating care for individuals with severe and enduring mental health needs.
As a Community Care Professional, you will work closely with local specialist mental health teams, tertiary service providers, primary care, and other agencies to provide comprehensive and innovative services to our service users and their loved ones.
Main Duties of the Job
Key responsibilities include:
- Assessing and planning care for individuals with complex mental health needs
- Coordinating and delivering care services in collaboration with other healthcare professionals
- Evaluating the effectiveness of care services and making recommendations for improvement
