About the Role

We are seeking a skilled and compassionate Support Specialist to join our Higher Needs Floating Support Service. As a key member of our team, you will work closely with clients who face multiple disadvantages and complex needs, providing tailored support to help them achieve their goals and sustain their accommodation.

Key Responsibilities

  • Work collaboratively with clients to identify and address their needs, developing a comprehensive support plan to promote their independence and well-being.
  • Build and maintain relationships with local authorities, healthcare providers, and other stakeholders to ensure seamless support and advocacy for our clients.
  • Provide outreach and floating support services to clients in temporary or settled accommodation, working to sustain their current living situation and prevent homelessness.
  • Develop and maintain a network of support for each client, including landlords, support agencies, and community resources.
  • Conduct regular reviews and assessments to ensure the effectiveness of support plans and make adjustments as needed.

Person Specification

  • Proven experience in service provision for people with complex and multiple needs, particularly in areas such as mental health, drug and alcohol, homelessness, and offending.
  • Experience in outreach or floating support work, with a strong track record of successfully engaging clients and promoting their independence.
  • Ability to build strong relationships with clients, colleagues, and external partners, working collaboratively to achieve shared goals.
  • Proven ability to promote and apply good practice in equality and diversity, inclusion, and risk management.
