Job Summary:
We are seeking a highly skilled and experienced Complex Needs Practitioner to join our team at Recovery Focus. As a key member of our team, you will be responsible for providing vital support services to individuals with complex needs related to addiction and substance misuse.
About the Role:
This is a permanent full-time role requiring the post holder to work 37 hours per week. You will be working flexibly across the community and within multi-agency partnership settings on an outreach basis, engaging people in a variety of interventions using a person-centred approach.
Key Responsibilities:
- Providing screening, co-produced assessment, psychosocial interventions, integrated and outcome-focused care planning, after-care support, and onward transition to other services as required
- Managing a caseload of complex cases requiring a case co-ordination approach, completing all recording and case management administration accurately and promptly
- Liaising with voluntary and statutory agencies, health professionals, and other stakeholders to ensure effective service delivery
- Developing and maintaining strong relationships with clients, families, and communities to promote positive outcomes
- Working collaboratively with the team to achieve service goals and objectives
Requirements:
- Significant experience of engaging effectively with clients in an addictions or social care setting
- Knowledge of primary, secondary, and other outreach settings, as well as the functional model
- Excellent communication and interpersonal skills, with the ability to work with diverse groups and individuals
- Good IT skills, particularly Microsoft Office, and a willingness to travel throughout the area
- A driving licence and access to a car is desirable
