We are seeking a highly skilled Senior Social Care Professional to join our team at Ackerman Pierce in Hounslow. The successful candidate will work within the Hospital Discharge Team, providing exceptional care to vulnerable adults in a hospital setting.

Key Responsibilities:

  • Develop and implement effective care plans to meet the needs of service users.

Requirements:

  • Post-qualifying experience as a Senior Social Worker in the UK Social Care industry.
