About the Role
As a Community Support Specialist, you will play a vital role in supporting adults in Leicestershire to live independently and promote their well-being. You will work closely with individuals, families, and carers to understand their needs and develop personalized plans to address their challenges.
Key Responsibilities
- Conduct assessments and develop care plans to support individuals in achieving their goals
- Work collaboratively with other agencies and organizations to ensure seamless support and services
- Provide guidance and support to individuals and their families to help them navigate the social care system
- Identify and report potential safeguarding concerns and take appropriate action
- Develop and maintain relationships with community partners and stakeholders to promote the benefits of supported living
Requirements
- Hold a Level 2 qualification in Maths and English
- Have relevant experience working with adults in a social care setting and knowledge of supported living and tenancies
- Be able to recognize the limits of own accountability and responsibility, and seek appropriate support and advice when required
- Be able to manage a caseload and work to clear objectives
- Be a confident communicator with excellent negotiation, persuasion, and conflict management skills
