About the Role

We are seeking a highly motivated and caring Mental Health Practitioner to join our Community Older Adults Mental Health Team in Wokingham. As a key member of our team, you will be responsible for assessing and implementing intensive care and treatment for older adults with functional mental health problems and dementia.

The successful candidate will have a passion for working with older people and a strong understanding of the Care Act, Mental Capacity Act, and Mental Health Act. You will be responsible for managing your own caseload, monitoring mental health needs, and working with carers and other professionals to assess care needs.

Main Responsibilities
  • Assess and implement intensive care and treatment for older adults with functional mental health problems and dementia.
  • Manage your own caseload, monitoring mental health needs and working with carers and other professionals to assess care needs.
  • Provide specialist consultation and advice on issues relating to the care of older people with mental health problems to other professionals within the wider care community.
