About the Role

We are seeking a highly skilled and compassionate Adult Mental Health Practitioner to join our Community Mental Health Team at Prospect Park Hospital in Reading.

As a key member of our team, you will be responsible for providing high-quality care and support to patients with mental health needs, working collaboratively with other healthcare professionals to deliver outstanding patient outcomes.

The successful candidate will possess excellent communication and interpersonal skills, with the ability to work flexibly and reliably in a fast-paced environment.

Key Responsibilities
  • Assess, plan, implement, and evaluate care for patients with mental health problems.
  • Work collaboratively with other healthcare professionals to deliver high-quality patient care.
  • Develop and maintain effective relationships with patients, families, and carers.
  • Contribute to the development and implementation of care plans that meet the individual needs of patients.
  • Participate in multidisciplinary team meetings to discuss patient care and progress.
Requirements
  • Registered Mental Health Practitioner with a degree in health/social care or equivalent qualification/experience.
  • Excellent knowledge and understanding of the health and social care systems and operating frameworks.
  • Ability to empathise, be supportive, and sensitive to the needs of others.
  • Competent IT skills and the ability to navigate around various systems and software packages.
  • Ability to work flexibly on a shift pattern as required and travel independently between locations.
