About the Role

We are seeking a highly motivated and caring Mental Health Practitioner to join our Community Older Adults Mental Health Team. As a key member of our team, you will be responsible for assessing and implementing intensive care and treatment for older adults with functional mental health problems and dementia.

Key Responsibilities
  • Manage a caseload of older adults with mental health needs, monitoring their mental health and working with carers to assess care needs under the Care Act.
  • Provide specialist consultation and advice on issues relating to the care of older people with mental health problems to other professionals within the wider care community.
  • Monitor and evaluate agreed care plans, making adjustments as necessary in collaboration with the wider team.
Requirements
  • Registered Mental Health Practitioner (Nurse, OT, Social Worker, Speech and Language Therapist)
  • Relevant clinical experience and passion for working with older adults
  • Valid UK driving license and ability to travel to various settings
