About the Role

We are seeking a highly motivated and enthusiastic Mental Health Practitioner to join our Community Older Adults Mental Health Team in Wokingham. As a key member of our team, you will be responsible for assessing and implementing intensive care and treatment for older adults with functional mental health problems and dementia.

This is a developmental role, offering the opportunity to progress to a B6 Mental Health Practitioner following a period of 18 months in supported practice and successful demonstration of competence.

We are committed to providing high-quality care to our patients, and we are looking for someone who shares our passion for working with older adults and has a strong understanding of the care of people with functional illness and dementia.

Key Responsibilities
  • Manage a caseload of older adults with mental health needs, working closely with carers to provide education and support.
  • Monitor and evaluate care plans, making adjustments as necessary in collaboration with the wider team.
  • Provide specialist consultation and advice on issues related to the care of older adults with mental health problems to other professionals within the care community.
