About the Role

We are seeking a highly skilled and experienced Mental Health Specialist Practitioner to join our team at Berkshire Healthcare NHS Foundation Trust. As a key member of our primary care mental health team, you will play a vital role in delivering high-quality care to individuals with complex mental health needs.

Key Responsibilities
  • Work collaboratively with multidisciplinary teams to assess, plan, and implement care for patients with mental health problems.
  • Provide a range of evidence-based interventions to individuals with severe and significant mental illness.
  • Conduct physical health assessments and develop holistic health and social care plans.
  • Work effectively in partnership with primary care and community/voluntary sector organizations to deliver integrated health and social care outcomes.
  • Participate in clinical audits and research to improve service delivery and patient outcomes.
Requirements
  • Current professional registration as a Registered Mental Health Nurse, Social Worker, Occupational Therapist, or equivalent.
  • Evidence of relevant post-registration training and development in motivational interviewing, brief solution-focused interventions, cognitive and behavioral approaches, and psychosocial interventions.
  • Minimum 5 years post-registration experience in working with individuals with mental health needs, including community experience in mental health services for adults and older adults with severe/significant mental illness.
  • Experience of providing a range of evidence-based interventions to individuals with a variety of mental health problems and severe mental illness.
  • Considerable experience in the assessment and management of clinical risk.
  • Experience of effective collaborative working in partnership to deliver holistic health and social care outcomes.
Desirable Qualifications and Experience
  • Independent and supplementary prescriber.
  • Recognized postgraduate clinical or leadership qualification relevant to the role.
  • Mentor and mentorship training.
  • Experience of working in primary and secondary mental health services.
  • Experience in the planning and delivery of training and practice developments to staff.
