Salary


The salary for this position is £43,077 - £56,055 per annum, depending on experience.



Job Description


We are seeking an experienced Nurse to join our Community Nursing Service as a Community Healthcare Specialist. This is an exciting opportunity to develop your skills and contribute to the delivery of high-quality patient care in the community.



Main Responsibilities



  • To provide skilled nursing care to patients in their own homes, based on principles of best practice and clinical evidence.
  • To act as a Named Nurse with the support and supervision of the DN Team Lead.
  • To undertake comprehensive assessments of patients' physical, psychological, and social care needs using the Single Assessment Process.
  • To provide individualised and person-centred care plans that promote self-care and prevent unnecessary hospital admissions.
