Job Summary

The Integrated Care Partnership is seeking an experienced and skilled Practice Nurse to join our team. As a key member of our primary care team, you will be responsible for providing high-quality patient care and support to our patients.

Main Responsibilities
  • Provide assessment, screening, and treatment services to patients, including health education and advice.
  • Work collaboratively with general practitioners and other healthcare professionals to deliver patient care.
  • Conduct health screenings and provide vaccinations and immunizations as required.
  • Manage wound care and ensure the maintenance of equipment and stock levels.
  • Collect, store, and dispatch pathological specimens and perform investigatory procedures.
  • Support patients and colleagues in adopting sound infection prevention and control measures.
  • Identify, diagnose, and manage patients at risk of developing long-term conditions.
  • Work with other healthcare professionals to diagnose, monitor, manage, and treat long-term conditions.
  • Support patients to adopt health promotion strategies and apply principles of self-care.
  • Demonstrate sensitive communication styles to ensure patients are fully informed and consent to treatment.
Administrative and Professional Responsibilities
  • Participate in the administrative and professional responsibilities of the Practice team.
  • Maintain accurate patient records and ensure accurate completion of all necessary documentation.
  • Ensure collection and maintenance of statistical information required for regular and ad hoc reports, audits, and research programs.
  • Attend and participate in Practice clinical meetings on a regular basis.
  • Assist with the Practice philosophy, strategy, and policy and develop appropriate protocols.
Professional/Personal Development
  • Maintain continued education by attendance at courses and study days as deemed useful or necessary for continuous professional development.
  • Participate in an annual individual performance review and take responsibility for maintaining a record of own personal and/or professional development.
  • Take responsibility for own development, learning, and performance and demonstrate skills and activities to others who are undertaking similar work.
  • Support the development of others in order to maximize potential.

Health & Safety

The post-holder will assist in promoting and maintaining their own and others' health, safety, and security as defined in the Practice Health & Safety Policy, to include:

  • Using personal security systems within the workplace according to Practice guidelines.
  • Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks.
  • Making effective use of training to update knowledge and skills.
  • Using appropriate infection control procedures, maintaining work areas in a tidy and safe way, and free from hazards.
  • Reporting potential risks identified.
