Practice Nurse

We are seeking an experienced and skilled Practice Nurse to join our team at the Integrated Care Partnership. As a Practice Nurse, you will play a vital role in providing high-quality patient care and contributing to the success of our organization.
Key Responsibilities- Provide assessment, screening, and treatment services to patients, including health education and advice.
- Work collaboratively with general practitioners and other healthcare professionals to deliver patient care.
- Conduct health screenings and provide vaccinations and immunizations as required.
- Manage wound care and ensure the maintenance of equipment and stock levels.
- Collect, store, and dispatch pathological specimens and perform investigatory procedures.
- Support patients and colleagues in adopting sound infection prevention and control measures.
- Identify, diagnose, and manage patients at risk of developing long-term conditions.
- Work with other healthcare professionals to diagnose, monitor, manage, and treat long-term conditions.
- Support patients to adopt health promotion strategies and apply principles of self-care.
- Demonstrate sensitive communication styles to ensure patients are fully informed and consent to treatment.
- Participate in planning and implementing changes within the area of care and responsibility.
- Work collaboratively with colleagues, multidisciplinary teams, and other healthcare professionals.
- Participate in the administrative and professional responsibilities of the Practice team.
- Maintain accurate patient records of all consultations and treatments.
- Ensure accurate completion of all necessary documentation associated with patient healthcare and registration with the Practice.
- Ensure collection and maintenance of statistical information required for regular and ad hoc reports, audits, and research programs.
- Attend and participate in Practice clinical meetings on a regular basis.
- Assist with the Practice philosophy, strategy, and policy and develop appropriate protocols.
- Maintain continued education by attendance at courses and study days as deemed useful or necessary for continuous professional development.
- Participate in any training program implemented by the Practice as part of this employment.
- Participate in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development.
- Take responsibility for own development, learning, and performance and demonstrate skills and activities to others who are undertaking similar work.
- Support the development of others in order to maximize potential.

Information relating to patients, carers, colleagues, other healthcare workers or the business of the Practice may only be divulged to authorized persons in accordance with the Practice policies and procedures relating to confidentiality and the protection of personal and sensitive data.
Health & SafetyThe post-holder will assist in promoting and maintaining their own and others' health, safety, and security as defined in the Practice Health & Safety Policy, to include:
- Using personal security systems within the workplace according to Practice guidelines.
- Identifying the risks involved in work activities and undertaking such activities in a way that manages those risks.
- Making effective use of training to update knowledge and skills.
- Using appropriate infection control procedures, maintaining work areas in a tidy and safe way and free from hazards.
- Reporting potential risks identified.
