Position Overview

We are seeking a dedicated and skilled professional in the field of neurological rehabilitation. The ideal candidate will be a registered Nurse or Allied Health Professional responsible for:

Exhibiting expertise in person-centred evaluations. This role necessitates collaboration within a multi-agency framework to facilitate the assessment and review process for neuro rehabilitation funding.

The successful applicant will be tasked with fostering, monitoring, and sustaining strong working relationships with patients, their families, care providers, statutory health services, and specialized services.

Experience in conducting mental capacity and best interests assessments is advantageous. Familiarity with the Continuing Healthcare (CHC) process would also be beneficial for this position.

Proficiency in report writing is essential to ensure comprehensive reviews that support patients throughout their rehabilitation journey. This includes assisting patients in setting achievable goals that adhere to the SMART criteria.

Key Responsibilities

Under the supervision of the Neuro Rehabilitation Lead/Complex Case Manager, you will collaborate with the team to deliver a professional and effective service in accordance with NHS England's Rehabilitation Commissioning guidance, NICE guidelines, and Department of Health recommendations.

The post holder will assist the Neuro Rehabilitation Lead/Complex Case Manager in planning and managing their review workload effectively, taking the lead in ensuring successful outcomes.

Engage with patients, their families, and healthcare professionals to determine the appropriate neuro rehabilitation pathway tailored to individual needs. Collaborate with the case manager to commission necessary services on a personalized basis.

  • Support the Neuro Rehabilitation team in achieving the Directorate's objectives.

Conduct regular reviews for individuals on the caseload, manage expectations, and establish timelines for rehabilitation.

Prepare reports for the Personal Health Budget (PHB) panel to ensure ongoing funding throughout the rehabilitation process.

Collaborate with the case manager, patients, families, and clinicians to determine the conclusion of rehabilitation and outline appropriate next steps, including referrals for CHC funding if necessary.

Job Responsibilities

The successful candidate will be expected to work both independently and collaboratively within a team, demonstrating adaptability in a dynamic environment. Engage with multidisciplinary teams in secondary care, primary care, nursing homes, etc., to facilitate the neuro rehabilitation process.

Establish and maintain effective communication networks with health and social care professionals.

Interpret national and regional guidelines related to rehabilitation, with a particular focus on Neuro Rehabilitation. Implement policies and recommend changes to practices and procedures as necessary.

Exercise professional judgment when conveying information.

Collaborate with relevant health professionals, Continuing Healthcare colleagues, commissioning teams, and other key stakeholders to ensure the achievement of key performance indicators.

Facilitate seamless patient flow throughout all stages of the patient journey, including coordination with primary care, secondary care, community venues, and specialized services.

  • Support the organization's development in the Integrated Personal Commissioning agenda and assist in the delivery of Personal Health Budgets.

Ensure data systems are maintained in accordance with standard operating policies and procedures.

Comply with the Data Protection Act and confidentiality guidelines.

Participate in ongoing professional development and clinical supervision.

Ensure compliance with all statutory and mandatory training requirements.

Maintain professional registration and ensure revalidation is up to date.

Uphold professional and appropriate communication at all times.

  • Foster constructive relationships with a wide range of internal and external stakeholders.

Collaborate closely with all care providers involved in service delivery.

Coordinate and participate in relevant meetings, providing information, advice, and support as needed.

Ensure that collected data is reported appropriately and monitor the processing of information.

Provide timely and relevant specialist advice and guidance on functional and informational matters related to Neuro Rehabilitation and associated services.

Maintain accurate and legible records of information.

Assist individuals and their representatives in navigating the NHS Neuro Rehabilitation process.

  • Enhance current management information systems to support decision-making processes.

Achieve objectives and performance targets, ensuring quality outcomes while prioritizing workload and meeting deadlines.

Support the training and induction of new staff members.

Engage in research and development activities as directed.

Qualifications

Essential

  • Degree-level education in a relevant field or equivalent experience in a similar specialist area, preferably in Neurological Rehabilitation.
  • Registered Allied Healthcare Professional with HCPC or NMC registration.
  • Evidence of post-qualification and ongoing professional development.

Knowledge and Experience

Essential

  • Understanding of the Caldicott principles and information governance.
  • Experience collaborating effectively with clinicians, social care, and partner agencies.
  • Demonstrated knowledge of safeguarding principles and processes.
  • Familiarity with confidentiality and the Data Protection Act.
  • Ability to demonstrate understanding of the Mental Capacity Act and consent processes.
