About the Role
We are seeking a dedicated and enthusiastic Specialist Occupational Therapist to join our newly developed Integrated Community Stroke Service within Sherwood Forest Hospitals NHS Foundation Trust.
The Stroke Early Supported Discharge team, established in 2009, has expanded into an integrated Community Stroke Service from April 2023. This development allows us to provide more accessible specialist stroke community-based rehabilitation across Mansfield, Ashfield, Newark, and Sherwood.
Our high-profile service focuses on facilitating a smooth transition for patients from hospital to home, optimizing their stroke recovery, function, and independence within the first six months post-discharge.
We prioritize patient-centered care and maintain high clinical standards. We are looking for a Specialist Occupational Therapist with a genuine interest in stroke care and rehabilitation who is eager to take on new challenges and pursue opportunities for personal and service development.
Main Responsibilities- Conduct comprehensive assessments of patients' needs following an acute stroke.
- Develop and implement individualized treatment plans to promote functional independence and enhance quality of life.
- Provide hands-on therapy interventions, including activities of daily living (ADL) training, adaptive equipment recommendations, and home safety evaluations.
- Collaborate effectively with a multidisciplinary team, including physicians, nurses, physiotherapists, and social workers, to ensure holistic patient care.
- Educate patients and their families on stroke recovery strategies, self-management techniques, and community resources.
- Maintain accurate documentation of patient progress and treatment interventions.
You are a highly motivated and compassionate Occupational Therapist with proven experience in working with patients recovering from stroke. You possess excellent communication, interpersonal, and clinical skills. Your commitment to providing high-quality care aligns with our values of patient-centeredness, teamwork, and continuous improvement.
