About the Role

The Specialist Team for Early Psychosis (STEP) is seeking a dedicated and experienced Community Mental Health Practitioner to join our team in Torbay, South and West Devon. This role focuses on providing comprehensive support to individuals experiencing their first episode of psychosis.

Your Responsibilities

  • Conduct thorough assessments and develop personalized care plans for individuals and their families/carers navigating a first episode of psychosis.
  • Collaborate effectively within a multidisciplinary team, including social workers, psychologists, and prescribing specialists.
  • Engage with service users and their families in the community, building strong therapeutic relationships to prevent hospital admissions.
  • Deliver evidence-based interventions aligned with NICE guidelines, incorporating family or systemic work as appropriate.
  • Provide psychoeducation about first episode psychosis to both service users and their families.
  • Address basic needs such as housing, income support, and access to essential resources.
  • Work proactively to manage a caseload of clients with diverse mental health challenges.
