Key Responsibilities- Conduct comprehensive assessments and develop personalized care plans for patients with complex mental health needs.
- Provide expert clinical guidance and support to patients, families, and carers.
- Collaborate with multidisciplinary teams to ensure seamless care coordination and effective communication.
- Develop and maintain strong relationships with external agencies and stakeholders to ensure comprehensive care.
- Participate in clinical governance and quality improvement initiatives to ensure high-quality patient care.
- Supervise and mentor junior colleagues to ensure effective knowledge transfer and skill development.
- Stay up-to-date with best practices and evidence-based guidelines to ensure delivery of high-quality patient care.
- Registered Mental Health Professional (RMN, OT, or Social Worker) with a strong understanding of mental health legislation and policies.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and multidisciplinary teams.
- Ability to work independently and as part of a team, with a strong focus on patient-centered care.
- Strong clinical assessment and problem-solving skills, with the ability to prioritize tasks and manage a high-volume caseload.
- Ability to work flexibly and adapt to changing priorities and circumstances.
