About the Role

We are seeking a highly skilled and dedicated Mental Health Practitioner to join our team at the NHS. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality mental health services to individuals and their carers.

Key Responsibilities
  • Assessment and Treatment: Conduct comprehensive assessments and develop personalized care plans to ensure the best possible outcomes for our service users.
  • Communication and Collaboration: Effectively communicate with service users, carers, and multidisciplinary teams to ensure seamless care coordination and delivery.
  • Risk Management: Identify and mitigate risks associated with mental health presentations, ensuring the safety and well-being of our service users.
  • Care Planning and Coordination: Develop and implement care plans that address the physical, emotional, and social needs of our service users.
  • Service Development: Contribute to the development and implementation of mental health services that promote recovery, social inclusion, and community engagement.
  • Supervision and Training: Provide clinical supervision and training to junior colleagues, ensuring the highest standards of care and practice.
  • Quality Improvement: Participate in quality improvement initiatives to ensure the delivery of high-quality, patient-centered care.
Requirements
  • Qualifications: An appropriate professional mental health qualification and registration (e.g., RMN, OT, or Social Worker).
  • Experience: Proven experience of working with adults with mental health problems, preferably in a community setting.
  • Skills: Excellent communication and interpersonal skills, with the ability to work effectively with diverse groups and individuals.
  • Knowledge: Understanding of recovery principles, mental health legislation, and the NHS Constitution.
