Job Overview

The Swansea Bay Acute Clinical Team is a dynamic, nurse-led service supported by consultant geriatricians, providing hospital-based treatment in the patient's home. The team operates two hubs, one based at Bonymaen Clinic and the other at Cimla Health and Social Care Centre, covering vast geographical areas.

Main Responsibilities
  • Assess and manage acutely unwell adult patients in the community environment.
  • Provide hospital-based treatments in the patient's home.
  • Work collaboratively with advanced clinical practitioners and geriatricians to develop treatment plans.
  • Communicate effectively with patients, families, and healthcare professionals.
  • Ensure high standards of care are maintained at all times.
Requirements
  • Hold a full UK driving license and have access to a vehicle for work purposes.
  • Have experience caring for adults in an acute setting.
  • Be able to recognize and manage deteriorating patients.
  • Administer prescribed treatments and monitor their effects.
  • Be flexible and able to adapt to changing service needs.
