Senior Medicines Management Facilitator

Demonstrate a sound understanding of clinical governance and risk management identifying implications for self, service users and colleagues. Ensure the use of evidenced based practice in line with national, local and specialist guidelines. Maintain and promote awareness of current developments and their implications for the Medicines Management Service. Maintain a professional portfolio for CPD recording learning outcomes through participation in internal and external development opportunities.
Responsibility for Patient/client Care Use highly specialist clinical knowledge to work across Health and Social Care to improve medicines management, promote independence and improve care outcomes for individuals requiring support in their own homes. Undertake individual assessments of service users and, building on the persons strengths, identify needs with respect to their medicines, optimizing independence where possible. Ensure effective liaison with colleagues across health and social care to establish a suitable level of assistance, supporting more generic staff in the delivery of this assistance. Demonstrate specialist knowledge to provide individual support for service users with complex needs including medication review and suggestions for alteration of medicines regime in collaboration with the service users doctor where appropriate.
Demonstrate a sound understanding of the needs of older and disabled people, building on strengths and promoting independence with regards to medicines management, advising on appropriate aids and systems to support independence and achieve personal outcomes. Provide specialist medicines management advice for minimisation of harm, improving quality, promoting cost effective prescribing, and best clinical outcomes. Where appropriate (and within competency): provide advice on changes to medicines regimes, dosage, formulations etc highlight basic drug monitoring requirements to prescribers undertake simple physiological measurements, basic pain and/or cognitive evaluations that support medicines management assessments or decisions Involve service users, families and carers in the assessment, planning and evaluation of identified medicines management needs. Identify issues of risk by undertaking comprehensive risk assessment of the service user, their needs and their environment, whilst supporting positive risk taking where appropriate.
Provide health education to service users and their informal carers with regard to medicines management, including their storage, ordering, administration and identification of potential problems such as side effects. Monitor progress and ensure effectiveness of the intervention and record outcomes for all service users on caseload, audit outcomes and share with peers and members of the multidisciplinary team. Communication, Key Relationships and Partnership Working Work effectively across multidisciplinary and multi-agency boundaries with a key role in improving joint working and communication across Primary, Intermediate and Secondary Care and across Health and Social Care. Act as a lead communicator with community pharmacies with respect to medicines management for clients receiving domiciliary care.
Communicate effectively with key stakeholders such as clinicians, social services and the wider multidisciplinary team, to lead and influence the development and coordinating of medicines management services in domiciliary care, implementing a communication strategy. Demonstrate effective use of counselling skills when supporting service users/relatives during assessments providing complex sensitive, contentious information regarding medicines management plans and other healthcare issues. Demonstrate a high level of interpersonal and communication skills when dealing with distressing and challenging behaviour. Convey and receive complex sensitive information in an understandable form to service users, colleagues, and students where motivational, persuasive, empathetic and reassurance skills are required.
Provide reports when needed on all aspects of the service including service user outcomes, training and clinical governance issues to the Health Board and Social Services. Be involved in the production and distribution of information fact sheets, educational material and prescribing newsletters for GPs, nurses, care workers, pharmacy and practice staff and to enable them to increase clinical knowledge and provide better care for service users. Provide expert advice to Health, Social and Third sector colleagues on medicines management issues to ensure the effective delivery of medicines related care packages. Working proactively to improve service users outcomes and minimize hospital admissions.
