Co-occurring Mental Health and Substance Use Link

The role of the Co-occurring Link Worker has been developed to improve outcomes for individuals with co-occurring mental health and substance use needs.
You will work across both Community Substance Use services and Community Mental Health Services to provide training, support and advice to professionals working with people with co-occurring needs as well as providing strong communication links and shared joined up approach to working with individuals.
Work as a member of the Community Drug and Alcohol Team and liaise effectively with Community Mental Health services, other service providers and stakeholders including housing, probation, social services and non-statutory services.
To support the MDT in delivering safe, effective treatment for patients with substance use and mental health needs.
Provide treatment for individuals with co-occurring substance use and mental health issues following NICE Guidelines and local pathways.
Educate and provide specialist advice and guidance to the multi-disciplinary team on the management of both mental health and substance use issues.
Provide advice on new referrals into services for clients with co-occurring needs and sign post if appropriate.
To be part of a culture that focuses on improving outcomes across the care pathway for patients, and joint working with CMHT and CDAT colleagues.
Manage workload effectively and responsibility for planning and delivering of individuals care and discharge process.
The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.
