Senior Nurse in Community Care

Canterbury, Kent, United Kingdom Kent Community Health NHS Foundation Trust Full time

Job Overview

The landscape of healthcare is evolving, with a growing focus on community-oriented services. This is an excellent opportunity to become part of a dedicated team.

We are currently seeking compassionate healthcare professionals who prioritize patient interaction. You will have the opportunity to treat individuals holistically, addressing their overall well-being rather than just their medical conditions. Your role will be pivotal in helping patients achieve their personal health goals.

We invite applications for the position of Clinical Lead Nurse at our esteemed Community Hospital. Our facilities are led by nursing professionals and emphasize comprehensive rehabilitation and therapeutic support for individuals requiring additional assistance to return home or those in palliative care.

While the environment is supportive and nurturing, it also demands quick thinking and the ability to make independent decisions in the best interest of your patients, under the guidance of the supervising nurse.

Main Responsibilities

As a Clinical Lead Nurse, you will play a vital role in delivering high-quality care within an inpatient setting. Your contributions will be essential to the effective operation of our hospital and the recovery of our patients.

Your responsibilities will include:

  • Collaborating with a multidisciplinary team to provide exceptional care to patients and their families
  • Monitoring and evaluating the quality of care provided
  • Providing constructive feedback to management regarding care standards

Your patient demographic may vary, often including elderly individuals, those recently discharged from acute care, or patients with terminal illnesses or physical disabilities.

As a Clinical Lead Nurse, you will ensure that care plans are effectively implemented to support patients during their stay, ensuring a positive experience for all service users.

Working for Our Organization

Our organization has received an 'outstanding' rating from the CQC, reflecting our commitment to delivering exceptional patient care and services that enhance community health. We seek outstanding individuals who share our core values: Compassionate, Aspirational, Responsive, and Excellent.

We recognize that a commitment to equity, diversity, and inclusion is essential for optimal patient care and a fulfilling workplace. We welcome applications from individuals with diverse backgrounds, perspectives, and experiences.

We encourage you to discuss any flexible working arrangements that may accommodate your personal interests or family commitments. We are dedicated to supporting a work-life balance that benefits both our patients and our staff.

Detailed Job Description and Responsibilities

The job description and person specification outline all necessary information regarding this role. Please review the criteria carefully and provide examples in your application that demonstrate your qualifications. If you require the application form in a different format, please inform our recruitment team.

We encourage inquiries regarding this position, and the recruiting manager is available to address any questions you may have.

At our organization, we strongly advocate for full vaccination among colleagues in clinical and non-clinical roles who have direct contact with patients, ensuring the safety of our workforce and the individuals we serve.
