Our objective within the Community Reablement Team is to foster independence and enhance the daily experiences of patients and their families within the community.

We are excited to present a unique opportunity for a passionate and forward-thinking Occupational Therapist to become a part of our vibrant and supportive team.

The role emphasizes collaborative practice, particularly in service enhancement and team development. Our team is dedicated to assisting individuals in maintaining their independence at home, thereby minimizing the need for additional healthcare services.

The team comprises a variety of professionals, including occupational therapists, physiotherapists, dietitians, speech and language therapists, rehabilitation technicians, support staff, social workers, community wellness officers, nurses, healthcare support staff, and administrative personnel.

Key Responsibilities

This position will be situated within Integrated Health and Social Care teams.

The role offers the chance to work as part of a multidisciplinary team, providing rehabilitation services to individuals and groups in various environments, including reablement facilities, private residences, and care establishments.

The successful candidate will contribute to the innovative Home First initiative, facilitating timely discharges from healthcare facilities to allow patients to recuperate in their own homes.

Flexibility in work hours is essential, and participation in weekend services will be expected.

Proficiency in Welsh is advantageous for this position; however, applications from both English and Welsh speakers are encouraged.

Job Requirements

Candidates should demonstrate substantial experience in rehabilitation or reablement within the NHS, social care, or a comparable community/clinical environment.

Previous experience in therapy within the NHS, particularly in elderly care, orthopedics, and general medicine, is highly desirable.

The ability to travel independently between various service locations is essential, and candidates must be able to do so efficiently.

The role will involve utilizing electronic patient records and some administrative tasks; therefore, strong IT skills and familiarity with healthcare or social care IT systems are crucial.

You will be expected to provide professional and clinical guidance to junior staff, students, and support personnel.

Support from senior therapists and clinical team leaders will be available to assist you in your role.

Qualifications

Essential

  • Degree in Occupational Therapy (or equivalent qualification)
  • Professional Registration with HCPC

Desirable

  • Membership in a professional body such as BAOT
  • Accreditation as a fieldwork supervisor for OT students

Experience and Knowledge

Essential

  • Experience managing a caseload autonomously, including assessment, intervention planning, and evaluation in relevant clinical areas
  • Understanding of the Occupational Therapy process, particularly concerning physical health conditions
  • Comprehensive knowledge of current best practices in Occupational Therapy for the designated client group
  • Awareness of Health and Safety regulations
  • Familiarity with Clinical Governance principles

Desirable

  • Previous supervisory experience
  • Experience in research and/or audit, with a focus on implementing evidence-based practices
