The Impact of Your Role

As a Community Support Specialist, you will be integral to our medium-high risk supported accommodation service, assisting vulnerable individuals who have faced homelessness and may have complex needs, including mental health challenges and substance misuse. Your collaborative approach will ensure the delivery of exceptional support services, focusing on customer engagement and co-production. You will facilitate connections between customers and external services, tailoring support to meet individual needs while managing a small caseload of more complex cases.

Key Responsibilities

  • Support the Service Manager in planning and operational activities.
  • Build and maintain relationships with local services and stakeholders to enhance customer access to community resources.
  • Participate in networking events to promote our service framework.
  • Ensure a consistent, asset-based support approach for all customers.
  • Document service delivery through reports and other necessary documentation.
  • Collaborate within a multi-disciplinary team to safeguard our customers.
  • Develop and coordinate group activities for service users.
  • Share expertise with colleagues to uphold best practices.
  • Adhere to health and safety policies to ensure customer security.
  • Maintain accurate records of activities.

Supporting Colleagues and Customers

  • Provide guidance and mentorship to team members.
  • Conduct assessments of customer needs and risks, agreeing on support levels.

  • Ensure customer safety by following safeguarding procedures.
  • Keep detailed and accurate customer records.
