Job Summary

We are seeking a skilled and compassionate Adult Community Support Specialist to join our team at Superior Healthcare. As a Community Healthcare Assistant in Complex Care, you will play a vital role in providing high-quality care to individuals with long-term health conditions in the comfort of their own homes.

Key Responsibilities
  • Provide personalized care and support to clients with complex needs, including those with physical and mental health conditions.
  • Work collaboratively with our in-house team of nurses and supervisors to develop and implement individualized care plans.
  • Conduct thorough assessments of clients' needs and preferences to ensure tailored support.
  • Deliver care and support services with a focus on promoting independence, dignity, and well-being.
  • Communicate effectively with clients, families, and healthcare professionals to ensure seamless care coordination.
Requirements
  • Minimum 6 months' experience in a professional care setting, preferably with adults/children requiring complex care.
  • Confident, positive interpersonal skills with leadership abilities.
  • Satisfactory enhanced DBS disclosure.
  • Full, clean UK driving license with access to your own vehicle.
