About the Role

We are seeking a highly skilled and compassionate Community Nurse to join our team at Somerset NHS Foundation Trust. As a Community Nurse, you will play a vital role in providing high-quality care to patients in their homes, managing complex health issues and collaborating with other healthcare professionals.

Key Responsibilities
  • Provide holistic care to patients in their homes, assessing their needs and developing individualized care plans.
  • Manage a range of complex health issues, including IV medication administration, palliative and end-of-life care, and wound management.
  • Collaborate with other healthcare professionals, including doctors, therapists, and social workers, to ensure comprehensive care.
  • Develop and implement care plans, taking into account the patient's physical and mental health needs.
  • Work autonomously, making clinical decisions and taking responsibility for patient care.
Person Specification

We are looking for a qualified and experienced Community Nurse who possesses the following skills and qualities:

  • Appropriately registered with the Professional body as per post requirements.
  • Relevant degree in Nursing or equivalent qualification.
  • Knowledge of current national policies affecting the NHS.
  • Ability to work autonomously and make clinical decisions.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively as part of a team.
