As a PCN Pharmacy Technician, you will play a crucial rolein supporting our clinical team by ensuring the safe, effective, and efficientuse of medicines. You will work closely with GPs, nurses, and other healthcareprofessionals to provide comprehensive pharmaceutical care to our patients.

Main duties of the job

Conducting medication reviews and providing advice on the safe and effective use of medicines. To act as a Technical Pharmacy Advisor to Practice Clinicians Supporting the management of repeat prescriptions and ensuring timely and accurate production. Assisting with the implementation of prescribing policies and guidelines. Providing support for the management of long-term conditions and chronic disease management. Ensuring compliance with legal and professional standards in all aspects of pharmacy practice. Participating in audits and quality improvement initiatives to enhance patient care. Offering advice and support to patients on medication-related queries.

About us

This role is based at Wells City Practice, part of the group of five practices making up West Mendip is a beautiful cathedral city with lots to offer, including the Mendip countryside, and within easy reach of Bath, Bristol, and the south coast.

You will be joining a team consisting of5 partners, 2 salaried GPs, a paramedic practitioner, nurses, and HCAs, all led by an accredited Practice Manager and supported by a full admin team. We are a training practice and regularly host GP Registrars, Med School students and even A level work experience placements. We believe this plays a large part in future proofing the NHS. We currently have 8,350 patients and area friendly, welcoming, and supportive practice team who are all passionate about Primary Care.

JOB DESCRIPTION PCN Pharmacy Technician

1. JOB SUMMARY

To provide specialist pharmaceutical technical services, professional advice and direct support to Practice GPs, Nurses, HCAs and Reception/Administrative Staff.

To liaise with patients, clinical and non-clinical staff, practice, hospital and community pharmacists and patient's carers to improve the pharmaceutical care of patients.

To develop the role of the Practice Pharmacy Technician to maximise support to all Clinical and Non-clinical practice staff to improve patient care in relation to all aspects of medicines and their impact

To provide professional specialist medicines advice to GPs, Practice Nurses, Community Pharmacists, Community Nurses and other health professionals to promote the safe, effective and high quality, cost effective use of medicines in all prescribing decisions for the benefit of the Practice's patients

2. MAJOR DUTIES AND RESPONSIBILITIES

To act as a Technical Pharmacy Advisor to Practice Clinicians

To advise Practice Management and other non-clinical staff concerning administrative aspects of medicines management as appropriate

To manage repeat prescription requests, resolving queries where possible and producing prescriptions for clinical review and signature

To address the following areas without the need to consult a GP/Clinical Pharmacist in each case (although clinical support and advice will always be available if required):

Review dates following medication reviews

Facilitate and document correct addition & removal of medications following receipt of a discharge summaries, secondary care and community clinic letters.

Tidying repeat medication lists including synchronising durations and removing medications not ordered within a defined period

Changing acute medications to repeat where appropriate- A new medication that a GP/Pharmacist Prescriber has commenced and there is subsequently evidence for the need for the item on a repeat basis and necessary tests/checks have confirmed this

Addition/Removal of Medication for Nursing homes under GP/Pharmacist Prescriber guidance

Co-ordinate District Nurse Medication and Dressing requests for the practice population

Co-ordinate medical device prescription requests, including requests for catheters, nutritional supplements/feeds and stoma care products.

To work in conjunction with the nominated Clinicians who support Nursing Homes in the area

Provide independent information to patients with regard to medicines and prescribing changes, initiating further support from other healthcare professionals where appropriate

To review practice stock management and develop systems to improve processes.

Analyse Practice prescription data to produce reports as requested

Participate in review and setting of policies relating to medicines management and prescribing

Implement initiatives to reduce waste of prescribed medicines

Support and influence the Practice Clinical, Management and Administrative Teams to implement procedural and service changes relating to prescribing to ensure best practice is adopted in accordance with local guidelines, legislation, directives and safety updates NICE, MRHA etc.

Compile and submit monthly claim to the PPA.

3. SUPERVISION RECEIVED

The post holder will be operationally line managed by the Practice Manager with clinical supervision provided by the prescribing lead GP.

4. SUPERVISION EXERCISED

The post holder has no day to day line management responsibility and does not directly manage any members of staff. However they are responsible for mentoring and supervising non-clinical staff processing repeat prescription requests.

5. KEY WORKING RELATIONSHIPS

Develop and maintain key working relationships with:

Practice GPs, Practice Nurses and HCAs

Patients and Carers where appropriate

Practice Management, Reception and other Administrative staff

Secondary Care Pharmacists

CCG Head of Medicines Management & Medicines Management Team

Community Nurses and other allied health professionals

Community Pharmacists and support staff

Qualifications

Essential

Qualified Pharmacy Technician registered with the General Pharmaceutical Council (GPhC). Completed or progressing on Primary Care Education Pathway

Desirable

Experience working in a primary care or community pharmacy setting. Strong communication and interpersonal skills. Ability to work independently and as part of a multidisciplinary team. Excellent organisational and time-management skills. Commitment to continuous professional development.
