About the Role

We are seeking a highly skilled and experienced Palliative Care Consultant to join our team at Hospice in the Weald. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality, patient-centered care to individuals with palliative needs.

Key Responsibilities
  • Provide advanced clinical leadership and expertise in palliative care, working closely with the medical consultant and other healthcare professionals.
  • Develop and implement clinical strategies to improve patient outcomes and experiences, in collaboration with the hospice's clinical governance team.
  • Conduct skilled nursing interventions, assessing and addressing complex issues related to quality of life, prognosis, and end-of-life decision-making.
  • Lead on independent prescribing, ensuring safe and effective medication management for patients.
  • Collaborate with external healthcare partners to provide palliative care services, promoting best practices and evidence-based care.
  • Develop and conduct training sessions, speaking at conferences, and writing articles for publication to share expertise and promote palliative care.
  • Contribute to the development of professional practice, promoting evidence-based care and audit, and ensuring high standards of data quality.
Requirements
  • NMC registration and independent prescriber status.
  • Advanced assessment skills and experience in palliative care.
  • Leadership experience in a palliative care environment, with a proven track record of service development and improvement.
  • Excellent communication and problem-solving skills, with the ability to work collaboratively with multidisciplinary teams.
  • Strong leadership skills, with the ability to motivate and inspire teams to deliver high-quality care.
