We provide care for patients with life-limiting conditions, emphasizing individualized support. Our mission is to improve quality of life and help patients achieve their personal care goals, addressing long-term conditions such as heart failure, respiratory diseases, frailty, and neurological disorders, as well as oncological diagnoses.

Job Duties

Scope & Purpose

Registered staff may engage across various services offered by Hospice in the Weald to:

  • Deliver exceptional palliative care to patients with terminal illnesses, including dementia, cancer, organ failure, and neurological conditions.
  • Serve as a practical and advisory resource for other professionals regarding palliative care, encompassing physical, psychological, social, spiritual support, and bereavement assistance.
  • Ensure patients and their families are informed about the full range of services available, enabling them to make educated choices about their care during this critical time.

Core Responsibilities

  • Provide compassionate, personalized, and holistic care to patients and their families.
  • Assess, plan, implement, and evaluate care pathways that align with the Hospice philosophy.
  • Review and update care plans and documentation, escalating concerns as necessary.
  • Recognize and manage palliative care emergencies effectively.
  • Administer medications, including injectables, tailored to patient needs.
  • Set up and manage syringe drivers as required.
  • Ensure the safe handling and management of controlled substances and clinical supplies.
  • Support new team members through orientation and conduct medication framework assessments.
  • Facilitate referrals for counseling and bereavement support for patients' families.
  • Collaborate with the multidisciplinary team to maintain a high level of risk awareness regarding self and patient safety.

Leadership & Management

  • Act as a mentor for new staff, students, and healthcare practitioners.
  • Receive training to become a Practice Supervisor or Assessor for student nurses, contributing to the development of future registered staff.
  • Promote a workplace culture that embodies the values of the Hospice.
  • Empower volunteers to embody the Hospice values in their roles and advance the organization's mission.

Communication & Teamwork

  • Identify when escalation of patient needs to senior staff is necessary and coordinate arrangements accordingly.
  • Be an active team player, providing support to colleagues through reflection, advice, and guidance.
  • Develop collaborative relationships with other care providers, including NHS Trusts.
  • Maintain accurate patient records to support clinical and medical staff.
  • Recognize when to work autonomously and when to seek guidance.
  • Adhere to NMC regulations and professional conduct codes.
  • Take responsibility for personal health and safety and that of the wider team.

Qualifications

Essential

  • Registered Nurse (Level 1) or Registered Paramedic (HCPC) with two years of post-registration experience.
  • Evidence of ongoing professional development.
  • Palliative care experience preferred but not mandatory.
  • Experience working in a multi-professional team and the ability to manage workload effectively under pressure.
  • Flexible and dynamic approach to work.
  • Ability to communicate sensitively with patients and care professionals in challenging situations.
  • Problem-solving skills and the ability to address issues proactively.
  • Computer literacy.
  • Confidence to work autonomously and coordinate care.
