As a member of our Community Outreach Team, you will serve as the primary contact for new community referrals and incoming service inquiries. You will play a crucial role in delivering palliative support and guidance to patients and their families in their preferred environments, including residential and nursing facilities. Your responsibilities will include participating in a clinician rota to provide telephone support, as well as conducting assessments in clinics and homes.

Scope & Purpose
Hospice in the Weald is dedicated to nurturing and developing its workforce. Primary Objectives
To collaborate within the interdisciplinary team in the Community Outreach Service, delivering high-quality care to patients with life-limiting conditions in the community.
To provide patients and their caregivers with the expertise of a dedicated palliative care service, encompassing symptom management, social, emotional, and spiritual support.
To offer guidance, assistance, and education to primary healthcare teams and other professionals regarding palliative care matters, ensuring appropriate medication administration during community visits and supporting other healthcare providers as needed.
To contribute to extending our hospice's reach, ensuring we have the right personnel delivering care at the right time.
You will provide clinical guidance and specialized support to other professionals on palliative care issues, playing a vital role in helping patients achieve their preferred care and end-of-life locations.
Key Duties
Expected Outcomes
- Conduct and analyze clinical histories in a clear and logical manner.
- Document accurately, confidentially, and legibly all relevant aspects of patient interactions, including history, examination results, differential diagnoses, and management plans.
- Present cases effectively to the multidisciplinary team, contributing to patient management.

- Interpret investigation results and coordinate with colleagues to order them appropriately.
- Develop knowledge of symptom management options and advise the primary healthcare team accordingly.

Qualifications
