Children's Diabetes Specialist Nurse

We are seeking a highly skilled and dedicated Children's Diabetes Specialist Nurse to join our multidisciplinary team at Somerset NHS Foundation Trust. As a key member of our team, you will play a vital role in providing high-quality care to children and young people with diabetes, working closely with our paediatric diabetes team to deliver education, guidance, and emotional support to improve health outcomes.
Main Responsibilities- Provide exceptional clinical care, aiming for good clinical outcomes and promoting the importance of maintaining emotional and physical health.
- Develop and deliver a quality program of education tailored to each child's developmental stage, ensuring they have the skills and knowledge to manage their diabetes effectively.
- Work collaboratively with the paediatric diabetes team, including consultant paediatricians, children's dietitians, and other healthcare professionals, to provide a specialist, family-centred, evidence-based service.
- Assist in providing advice, education, and support to hospital and community staff, as well as children with diabetes and their families, when in hospital, at home, away from home, and at school/nursery.
- Contribute to the development and implementation of policies and procedures to ensure the highest standards of care for children with diabetes.
- Work in collaboration with adult diabetes care teams to ensure a smooth transition from paediatric to adult care.
