Children's Diabetes Specialist Nurse

We are seeking a highly skilled and dedicated Children's Diabetes Nurse to join our multidisciplinary team at Somerset NHS Foundation Trust. As a Children's Diabetes Specialist Nurse, you will play a vital role in supporting children and their families in managing diabetes, delivering education, and providing ongoing emotional and clinical support to improve health outcomes.
Main Responsibilities- Provide high-quality clinical care, aiming for good clinical outcomes and promoting the importance of maintaining emotional as well as physical health.
- Develop and deliver a quality program of education tailored to each child's developmental stage, ensuring they have the skills and knowledge to manage their diabetes effectively.
- Work collaboratively with the paediatric diabetes team, including consultant paediatricians, children's dietitians, and other healthcare professionals, to provide a specialist, family-centred, evidence-based service.
- Assist in providing advice, education, and support to hospital and community staff, as well as children with diabetes and their families, when in hospital, at home, away from home, and at school/nursery.
- Contribute to the development and implementation of policies and procedures related to paediatric diabetes care, ensuring they are aligned with best practice and evidence-based guidelines.
- Participate in quality improvement initiatives, identifying areas for improvement and contributing to the development of solutions to enhance patient care and outcomes.
