About the Role
We are seeking a dedicated and enthusiastic Occupational Therapist to join our dynamic Care Direct Plus North Team, based in Barnstaple.
This role offers an opportunity to work with adults experiencing a wide range of disabilities and health conditions across North Devon. You will play a vital part in empowering individuals to maximize their independence and safety while reducing risks for themselves and their caregivers.
Your Responsibilities
- Conduct comprehensive clinical assessments to determine the needs of individuals within their homes or virtually.
- Develop and implement person-centered treatment plans that promote independence, safety, and well-being.
- Collaborate with multidisciplinary teams, including NHS professionals, social care providers, housing organizations, and equipment suppliers to ensure holistic support for individuals.
- Provide advice, guidance, and recommendations on a range of interventions, such as adaptive equipment, minor home modifications, rehousing options, and community resources.
- Maintain accurate records and documentation of assessments, interventions, and progress notes.
