About the Role

Aston-Mansfield is seeking a skilled Community Outreach Specialist to join our team in supporting the Mental Health Transformation Programme. As a key member of our team, you will play a vital role in delivering high-quality support services to individuals in need.

Key Responsibilities
  • Case Management: Hold a caseload and provide support work within the local community, being alert to the mental health needs of residents.
  • Support and Signposting: Provide support and signposting in various ways, as agreed upon with the PCN and wider partnership, including meeting patients face-to-face, working within the MDT, and referring to other mental health provision.
  • Group Facilitation: Facilitate groups, provide coaching, build trust, and motivate individuals in their progress.
  • Risk Monitoring: Ongoing monitoring of risk to build a positive but sensitive attitude to caseload.
  • Communication: Provide clarity to the MDT and individuals on their lists about how different areas of their need are being addressed.
  • Continuity and Reassurance: Provide continuity and reassurance to individuals on caseload.
  • Feedback and Evaluation: Provide feedback to the PCN team and upwards about gaps in service provision.
Requirements
  • Strong initial assessment, action planning, and goal setting skills.
  • Ability to network with relevant stakeholders and engage patients face-to-face and remotely.
  • Confidence in approach and inquisitiveness.
  • Collaborative working with multiple agencies to promote patient welfare and outcomes.
