About Our Team and Role

Mental health services in Warwickshire are delivered through integrated teams, managed by Coventry and Warwickshire Partnership Trust in collaboration with the Local Authority. Our team is a multi-disciplinary group of professionals including Psychiatrists, Psychologists, Nurses, Occupational Therapists, and Support Workers. We work closely with Health colleagues and utilize their data systems as part of our agreement with Coventry and Warwickshire Partnership Mental Health Trust.

We provide care to individuals in the North Warwickshire community facing severe and enduring mental health challenges. Our approach centers on the Recovery and Strengths model, empowering individuals to reach their full potential by addressing their health and social needs. This includes support with medication management, accessing work, education, and leisure opportunities, as well as managing daily mental health needs.

Your Responsibilities as a Social Worker

As a Social Worker in our team, you will manage a caseload of service users with long-term social recovery needs. You'll help them maintain their mental well-being and achieve their full potential within the community. Your responsibilities include:

  • Completing Community Care Act Assessments
  • Providing social work interventions
  • Contributing to the Duty rota, assessing new service users referred to secondary mental health care
  • Championing the Social Model within secondary mental health
  • Actively participating in safeguarding vulnerable service users
