About the Role

The Care and Support Professional will provide high-quality care and support to a lady with mental health issues in her own home. This will involve assisting with daily tasks such as health appointments, shopping, cleaning, cooking, and social activities.

Key Responsibilities

  • Support the lady to maintain her independence and promote her choice, dignity, and respect.
  • Deliver care and support services that meet the highest standards of quality and safety.
  • Build a strong and trusting relationship with the lady and her family.
  • Work collaboratively with other healthcare professionals to ensure comprehensive care and support.
  • Contribute to the development and implementation of care plans that meet the lady's individual needs.

Requirements

  • Reliable and friendly with excellent communication skills.
  • Ability to work independently and as part of a team.
  • Good understanding of mental health issues and their impact on individuals.
  • Experience of working with vulnerable adults.
  • Ability to work flexibly, including evenings and weekends.
