We are seeking an experienced Rehabilitation Case Manager to join our team at Active Care Group. As a key member of our team, you will be responsible for providing high-quality case management services to adults and children with ongoing needs.
Key Responsibilities- Advocate for clients' needs while building strong relationships with families, deputies, and litigation solicitors.
- Develop and implement individualized case management plans with specific goals and timeframes.
- Recruit, select, and manage a team of care/support workers for clients.
- Implement and monitor individualized support and rehabilitation plans, making adjustments as needed.
- Provide flexible working patterns to support teams, ensuring smooth service operation, including out-of-hours, weekends, and public holidays.
- Compile risk assessments and identify risk management strategies to ensure safety for all parties.
- Hold a UK professional health qualification requiring registration with HCPC, NMC, or Social Worker.
- Full driving license and access to a car.
- Previous experience as a Case Manager, preferably supporting adults or children, with a full induction and training provided.
- Thorough knowledge of CQC fundamental standards and key lines of enquiry.
- Working knowledge of the Mental Capacity Act (MCA), Deprivation of Liberty Safeguards (DoLS), and Safeguarding frameworks.
- Ability to address and respond to employee issues in a fair and consistent manner.
- Strong organizational, time management, and planning skills.
- Flexible, professional, and positive approach to multi-site working.
