Job Overview

We are seeking a highly skilled and experienced Band 6 Team Leader to join our Community Nursing Team based in Deal. As a key member of our team, you will be responsible for leading and managing a caseload of patients with complex needs, providing holistic assessments, and delivering clinical interventions to meet their individual requirements.

Main Duties

As a Band 6 Team Leader, you will:

  • Undertake comprehensive assessments of patients' needs and develop personalized care plans.
  • Lead and manage a team of healthcare professionals to ensure high-quality patient care.
  • Collaborate with other healthcare teams to ensure seamless care delivery.
  • Provide health education and promote independence in patients to manage their complex long-term conditions.
  • Work closely with patients, families, and carers to ensure their needs are met and their expectations are exceeded.

Person Specification

We are looking for a highly skilled and experienced healthcare professional with:

  • Registration with the NMC as a healthcare professional.
  • Evidence of relevant postgraduate development at Level 6.
  • Specialist qualifications relevant to the area of practice.
  • Minimum of 5 years' experience in the area of practice.
  • Practical experience in managing health-related projects and complex caseloads.
  • Experience of leading and managing teams.
  • Valid driving license in the UK and access to a vehicle.
