About the Role

We are seeking a highly skilled and experienced Clinical Lead to join our Community Nursing Team in Deal. As a Band 6 Team Leader/Caseload Manager, you will be responsible for undertaking holistic assessments, planning appropriate care, and delivering clinical interventions to meet the needs of patients on your caseload.

You will work closely with a close-knit team to promote health and wellbeing in the community, offering health education advice and promoting independence in all consultations.

The successful candidate will have a valid driving licence and access to a vehicle, as well as experience of leading and managing teams, and the ability to work effectively with a variety of people at all organisational levels.

Key Responsibilities

  • Undertake holistic assessments and plan appropriate care for patients on your caseload
  • Deliver clinical interventions to meet the needs of patients
  • Work closely with the team to promote health and wellbeing in the community
  • Offer health education advice and promote independence in all consultations

Person Specification

  • Registered healthcare professional (NMC registration)
  • Health-related degree and evidence of relevant postgraduate development at Level 6
  • Experience of leading and managing teams
  • Ability to work effectively with a variety of people at all organisational levels
