About the Role

We are seeking a highly skilled and experienced Clinical Lead to join our Community Nursing Team in Deal. As a key member of our team, you will be responsible for leading and managing a caseload of patients, providing holistic assessments, and delivering clinical interventions to meet their needs.

You will work closely with our team to promote independence, health education, and support patients to manage their complex long-term conditions. Your expertise and leadership skills will be essential in driving high-quality patient care and contributing to the success of our team.

Key Responsibilities
  • Lead and manage a caseload of patients, providing holistic assessments and delivering clinical interventions
  • Develop and implement care plans to meet the needs of patients
  • Work closely with the team to promote independence, health education, and support patients to manage their complex long-term conditions
  • Contribute to the development and implementation of local and national agendas for health
  • Manage and lead change, prioritizing care delivery in a professional manner
Requirements
  • Registered healthcare professional (NMC registration)
  • Health-related degree and evidence of relevant postgraduate development at Level 6
  • Specialist qualifications relevant to the area of practice (e.g., non-medical prescribing, mentorship, coaching, clinical expertise)
  • PTLLS/DTLLS, PGCE, or Certificate in Education Management or supervisory qualification or experience of study
  • Minimum of 5 years' experience in the area of practice
  • Experience of leading and managing teams, using Microsoft Office and various IT packages
