Nursing Team Lead/Caseload Manager

We are seeking a highly skilled and experienced Band 6 Team Leader/Caseload Manager to join our Community Nursing Team based in Deal. As a key member of our team, you will be responsible for leading and managing a caseload of patients, providing holistic assessments, planning appropriate care, and delivering clinical interventions to meet the various needs of patients.
Main Duties of the JobAs a Band 6 Team Leader, you will:
- Undertake holistic assessments of patients and plan appropriate care to meet their needs.
- Deliver clinical interventions to patients on your caseload, including health education and advice to promote independence.
- Work closely with other healthcare professionals to ensure seamless care delivery.
- Manage and lead change, prioritizing care delivery in a professional manner, considering Trust policies and procedures.
We are looking for a candidate with:
- Registered healthcare professional (NMC registration) with a health-related degree and evidence of relevant postgraduate development at Level 6.
- Minimum of 5 years' experience in area of practice, including practical experience in managing health-related projects/complex caseloads and supporting others (mentor/educator/supervisor) experience.
- Valid driving license in the UK and access to a vehicle.
